Large Spigelian Hernia_Crimson Publishers Large Spigelian Hernia by Devaji Rao S in Surgical Medicine Open Access Journal Spigelian hernias form a minority of all abdominal wall hernias. They occur between the layers of the abdominal wall, i.e. between the transversus abdominis and the internal oblique muscles through a small slit like or oval defect and may become incarcerated. A variety of intra abdominal organs have been reported in the hernia sac. Obstruction and strangulation are potential complications but they are rare. The clinical diagnosis of a Spigelian hernia is difficult when it is small. Non Tuberculous Mycobacterium as a Causative Factor in Port Site Wound Infection - A Case Report by JS Rajkumar in Surgical Medicine Open Access Journal  Over the two decades there has been an increase in Non Tuberculous Mycobacterium (NTM) organisms in post laparoscopic port site skin and soft tissue infections. Presenting a patient who underwent eight successive surgical explorations of multiple skin and soft tissue sinuses and for aggressive necrosectomy and debridement of multiple sinuses caused by NTM organisms which ended up in abdominoplasty and meshplasty 2 months after the infection had been completely controlled. She finally had complete relief from the infection and had 6 months follow up from the last meshplasty surgery. It is a tertiary health care centre, for nephrology, with a well established hemodialysis unit. We have done about 1950 renal transplantation so far. Acute rejection is the most significant risk factor for chronic rejection and potential surrogate for longterm graft failure. Aim of our study was to analyze the association between the baseline through (C0) tacrolimus level in the first day post transplant, with early rejection in living donor transplants. Pancake Kidney: A Case Report_Crimson Publishers Pancake Kidney: A Case Report by  Chaker Kays in  Surgical Medicine Open Access Journal There are many developmental anomalies of the kidney. Pancake kidney is one of the rarest types of renalectopia. We report a case of pancake kidney which was detected incidentally while treating a male patient for renal colics. A CT scan concluded to pancake kidneys with right ureteropelvic junction obstruction, associated to multiple renal calculi.
